RFP.NO.HCOA.2026.BCBHC.01 Better Choices Better Health- Chronic Disease Self-Management Program Coordination

The County of Hawaii is seeking proposals from qualified applicants to provide coordination of the Better Choices Better Health (BCBH) – Ke Ola Pono Chronic Disease Self-Management Program (CDSMP). The program aims to continue development and implementation of the BCBH – Ke Ola Pono program throughout the County of Hawaii. The contract term will be from October 1, 2026, to September 30, 2027, and may be extended for up to one 12-month term. • The program is based on six workshop sessions, two and a half hours per session, held once a week for six consecutive weeks. • A six-month reunion session will be held after the last workshop session. • The program aims to complement and enhance medical treatment and disease management. • The applicant should have experience in administering programs and contracts for older individuals. • The applicant should demonstrate skills, abilities, and knowledge relating to the delivery of the proposed services. • The applicant should provide a detailed discussion of their approach to service activities and management requirements.
