Caregiver Support Programming

The South Dakota Department of Health is seeking an organization (or consortium) to design and deliver a statewide caregiver‑support program under the Rural Health Transformation initiative. The vendor will provide education, peer‑support groups, resource navigation, and training to reduce caregiver stress and improve health outcomes, reporting monthly on progress and expenditures. The contract runs for an initial 17‑month period with up to three one‑year renewal options and a total ceiling of roughly $450 K per year. • Design and implement caregiver education, support groups, and respite‑care referral pathways (virtual and in‑person). • Coordinate monthly progress reports, expenditure claims, and attend required virtual program meetings. • Collaborate with the Department of Health and the Department of Human Services, ensuring alignment with existing state caregiver initiatives. • Manage a budget up to approximately $637,500 for the first 17 months, adhering to a 6.3 % indirect‑cost limit and prohibiting participant incentives.
