• Provide unlimited‑lifetime‑reimbursement stop‑loss insurance (specific and aggregate) for UVU’s employee health plan. • Must cover both medical and pharmacy claims and mirror the underlying plan’s provisions. • Submit PEPM pricing via the Cost Proposal Template and answer detailed technical questions (laser handling, renewal caps, large‑claim notification, etc.). • Contract term: 1 year with up to four optional renewals; vendor must guarantee premium rates for the initial term. • Evaluation criteria include scope compliance (40 pts), cost (30 pts), experience (15 pts), financial stability (10 pts), and references (5 pts).
