Medical and Dental Insurance

The City of Celina is seeking proposals for health and dental insurance. The selected provider will be required to extend prices to all entities that have entered into an Interlocal Agreement with the City. The contract will be subject to acceptance for a period of 90 days. The City strongly requests that bidders submit responses electronically. • The proposal should include a complete breakdown of charges, including labor and mark-up on parts/materials. • The proposal should include a statement of qualifications, including the company's experience and qualifications. • The proposal should include a certificate of interested parties, filed electronically with the Texas Ethics Commission. • The proposal should include proof of insurance, which will be verified and tracked by the City. • The City maintains Interlocal Agreements with entities throughout the Dallas - Fort Worth area.
