Provision of Community Services

This contract is for the provision of community services, specifically the Primary Care Program, which aims to provide medical services to homeless and/or uninsured citizens of Orange County. The program will provide services such as patient visits, medical case management, primary care diagnosis and treatment, patient education, medication refill, labs, and xrays. The contract has a term of 3 years, with an estimated total funding of $1,076,568.01. The vendor, Health Care Center for the Homeless, Inc. DBA Orange Blossom Family Health, will provide the services and is required to maintain documentation to verify the provision of outputs/units of service reported. The contract also includes performance measures, which will be documented and reported upon bi-annually. Some key points to note include: • The program will serve at least 1,000 participants during the contract year. • The program will provide an estimated 1,306 units of service per year. • The unit rate for each service is $81.66 per visit. • The total funding for the contract is not to exceed $106,721.57 per year. • The contract includes a detailed scope of work, which outlines the specific services to be provided and the requirements for documentation and reporting. • The contract also includes a noncompliance standards section, which outlines the penalties for noncompliance with the contract terms.
