Description
This contract authorizes an independent contractor to provide unspecified services (detailed in Exhibit “A”, which is not included) to the Siskiyou County Health and Human Services Agency, Behavioral Health Division, for a one‑year period from July 1 2021 through June 30 2022. Compensation is limited to the amount appropriated by the Board of Supervisors and is paid based on services satisfactorily performed. The contractor must maintain required insurance (general liability, automobile, professional liability, workers’ compensation) with minimum $1 M limits, indemnify the County, and comply with all applicable California laws. The County retains the right to terminate for convenience, for funding shortfalls, or upon contractor bankruptcy/insolvency, and may require the return or secure destruction of all protected health information upon termination. All records must be retained for at least five years and are subject to County audit.
