Description

This Funding Opportunity Notice (NOFO) seeks cooperative agreements to develop and implement dementia‑capable home and community‑based service (HCBS) systems for American Indian/Alaska Native tribal communities. Up to six awards of $200,000‑$300,000 each will fund a 36‑month program that expands or creates dementia services for tribal elders and caregivers. Applicants must be federally recognized tribes, tribal governments, or tribal organizations/consortia that currently deliver HCBS. Two tracks are offered: (A) entities that have previously received ADPI funding and (B) new entrants to the ADPI program. Successful projects will deliver person‑centered, strengths‑based services that enable individuals with dementia to remain safe and independent in their communities. • Target audience: Native American tribal governments or tribal organizations (federally recognized). • Funding mechanism: Cooperative Agreement (grant‑like) with cost‑sharing required. • Total estimated funding: $1.7 million; award ceiling $300 k, floor $200 k. • Number of awards: 6. • Project period: 36 months, starting approx. 07/30/2027. • Key deliverables: Development of dementia‑capable HCBS infrastructure, training, caregiver support, and culturally appropriate service models. • Eligibility: U.S. entities only; foreign entities not eligible.

Budget estimate

$200,000 – $300,000

The NOFO indicates total estimated funding of $1,700,000 for six cooperative agreements, with each award ranging from $200,000 (floor) to $300,000 (ceiling). Assuming the full ceiling is awarded to all six, the maximum budget could be $1,800,000, but the stated ceiling suggests $300,000 per award is the upper bound. Therefore, a realistic budget range per award is $200k‑$300k, with the overall program budget around $1.2‑$1.8 million depending on award distribution. Factors influencing the final amount include the number of successful applicants (up to six), cost‑sharing requirements, and any adjustments made during award negotiations.
