Description

The Administration for Community Living (ACL) is issuing a Notice of Funding Opportunity (NOFO) under the Alzheimer’s Disease Programs Initiative (ADPI) to fund cooperative agreements that build dementia‑capable home and community‑based service (HCBS) systems. Two award options are available: (A) State‑level grants (36‑month agreements) for state agencies designated to lead dementia‑capable HCBS system development, and (B) Community‑level grants (36‑month agreements) for private or public community‑based organizations that already operate within a dementia‑capable HCBS system and can address three identified service gaps (support for persons living alone with ADRD, services for individuals with intellectual/developmental disabilities who also have ADRD, and behavioral symptom management training for caregivers). A total of 17 awards (approximately $15 million) will be made, with each award ranging from $850,000 to $1,000,000. No entity may hold more than one ADPI grant at a time, and priority will be given to states and organizations that have not received ADPI funding since before 2014. • Two award tracks: State (Option A) and Community (Option B) – both 36‑month cooperative agreements. • Funding: $850k–$1M per award; total anticipated funding $15M across 17 awards. • Eligible applicants: State governments, local governments, public housing authorities, nonprofits (501(c)(3) and non‑501(c)(3)), tribal entities, higher‑education institutions, etc., but an entity cannot apply for both tracks. • Focus areas: creation/expansion of dementia‑capable HCBS, single‑entry‑point/no‑wrong‑door access, and targeted service gaps for persons living alone, IDD populations, and caregiver training. • Application deadline: March 3, 2027; award decision expected July 30, 2027; project start July 30, 2027.

Budget estimate

$850,000 – $1,000,000

Each cooperative agreement is capped between $850,000 (award floor) and $1,000,000 (award ceiling). With 17 awards anticipated, the total program funding is about $15 million. Therefore, a successful applicant can expect to receive roughly $0.85 M–$1 M for a 36‑month project, depending on the scope of services proposed and the agency’s evaluation. Larger, state‑level projects may approach the ceiling, while smaller community organizations may receive amounts nearer the floor.
