Rural Health Transformation Program - Remote Patient Monitoring

The State of Vermont is seeking proposals from qualified organizations to provide Remote Patient Monitoring (RPM) technology and services to healthcare providers in Vermont. The purpose of this grant opportunity is to provide funds to provider organizations for the purchase or lease of RPM equipment and/or platform. RPM may be used in home health, longitudinal care programs, long term care, primary care/specialty, and community paramedicine settings. The grant period is anticipated to be 14 months, with an estimated start date of August 1, 2026. Some key points to consider: • RPM technology must be secure, HIPAA-compliant, and interoperable with federally Certified Electronic Health Record Technology (CEHRT). • Grant funds may not be used for broadband infrastructure or internet service. • Applicants must demonstrate compliance with the scope and requirements of this NOFO and will be evaluated on criteria such as qualifications, experience, and cost. • The State is seeking proposals from qualified organizations, including home health, longitudinal care programs, long term care, primary care/specialty, and community paramedicine practices physically located in Vermont. • Priority will be given to applicants serving a higher percentage of unique patients residing in rural ZIP codes.
