Enhancing Blueprint’s Transformation Network Capacity: Implementation Support via Clinical Champions

The State of Vermont is seeking proposals to enhance the capacity of Blueprint's transformation network. The purpose of the grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce, and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. The selected vendor will provide implementation support via clinical champions. The contract period is anticipated to be 14 months, with an anticipated start date of August 1, 2026. Key areas of focus include: * Team-based care * Chronic disease management * Integrated behavioral health/SUD care * Experience in practice transformation, population health, or QI leadership * Credibility as peer leaders within clinical communities The State will evaluate proposals based on the bidder's qualifications, experience, ability to meet the RFP minimum requirements, cost, and overall ability to produce deliverables.
