Blueprint for Health Quality Improvement Practice Facilitators

The Department of Vermont Health Access is seeking to establish contracts with one or more companies to provide Blueprint for Health Quality Improvement Facilitators. These facilitators will work with primary care practices, specialty care providers, and community collaboratives to implement quality improvement activities and support the implementation of the Blueprint for Health program. The facilitators will provide coaching, training, and technical assistance to help practices improve patient outcomes and experiences. The contract will be for a state-determined period of 1-2 years with options to renew for up to 4 additional years. • The Blueprint for Health is a comprehensive delivery system that has developed payment reforms, operational infrastructure, and an evaluation framework to support advanced primary care and community health. • The program has developed a team of trained professionals known as QI Facilitators to help practices build the capacity to improve care through evidence-based guidelines and quality improvement approaches. • The QI Facilitators will work with practices to implement programs and support continuous quality improvement activities, including creating infrastructure for continuous improvement, building skills in leadership and QI teams, and providing executive coaching to leadership. • The facilitators will also support the incorporation of Community Health Teams and other health and community services into practice workflow and participate in learning collaborative activities with assigned practices. • The contractor shall ensure that QI facilitation work includes leadership involvement and communication, aligning quality initiatives into the larger QI plan for the practice/organization, and encouraging/fostering practice ownership and support for Continuous Quality Improvement.
