Behavioral Health Nutrition Services

The State of Delaware’s Department of Health and Social Services (DHSS) is seeking a qualified vendor to provide behavioral‑health‑focused nutrition services for adults with serious persistent mental illness and co‑occurring substance‑use disorder. The vendor will conduct nutrition screening, deliver medical nutrition therapy (MNT) counseling, and provide group‑based nutrition education at Horizon House Recovery Community Center and at DSAMH Transitional Case Management sites. Services are to be delivered approximately six hours per week on‑site (plus documentation time) and must be reported monthly. The contract is a one‑year cost‑reimbursement award (renewable up to four optional one‑year extensions) with a maximum annual funding ceiling of $50,000. • Conduct food‑security, malnutrition, and chronic‑disease risk screenings using validated tools. • Provide individualized MNT counseling by Delaware‑licensed RDNs or licensed nutritionists, and coordinate referrals. • Deliver a structured group nutrition‑education curriculum (in‑person or virtual) covering nutrition’s role in mental health recovery and budget‑friendly meal planning. • Submit monthly program and usage reports, quarterly subcontractor reports, and an annual audit; comply with HIPAA, 42 CFR Part 2, state policies, and insurance requirements.
