Rural Health-Medicaid Primary Accountable Care Transformation (PACT)

The State of South Dakota’s Department of Social Services is seeking a vendor to design, develop, implement, and support a statewide Case Management Tool to support the Medicaid Primary Accountable Care Transformation (PACT) program across rural, tribal, and frontier health‑care providers.
