Speech-Language Pathology Services

The State of Wyoming, Department of Health, Behavioral Health Division, Wyoming Life Resource Center is seeking a contractor to provide speech-language pathology services to the residents at the Wyoming Life Resource Center. The contractor will be responsible for providing therapists in good standing, licensed in the State of Wyoming, to attend to residents' personal needs, assessments, care plan development, treatment of patients, and participation in interdisciplinary treatment team meetings. The contract will be for a period of up to 4 years. • The contractor must have experience in providing speech-language pathology services, with at least three project references for current or recently completed contracts similar in nature to the services required by the Agency. • The contractor must provide an organization chart with all proposed personnel, including the supervisor level, functional responsibilities, key personnel, and other staff members who will be involved in the project. • The contractor must submit a work plan that meets the needs of the RFP and indicates a thorough understanding of the scope of work. • The contractor must describe its approach to meeting the mandatory requirements and specifications as described in the RFP. • The contractor must provide an all-inclusive base hourly rate, an overtime rate, and a holiday rate for each position.
