The Commonwealth of Kentucky, through its Department of Veterans Affairs, is seeking qualified physicians (or qualified entities) to provide comprehensive primary‑care services to residents of the Carl M. Brashear Radcliff Veterans Center, a 120‑bed long‑term care facility. The contract will cover scheduled and acute medical encounters, admission assessments, discharge planning, on‑call coverage, and coordination of care with Medicare, Medicaid, and VA regulations. Proposals must include a detailed technical solution, cost solution, and optional oral presentation.
