The State of Delaware Department of Health and Social Services is seeking a professional‑services vendor to implement the National Cardiovascular Health Program (NCHP), a CDC‑cooperative‑agreement effort aimed at improving hypertension and cholesterol outcomes statewide. The work is organized around three mandatory strategy areas—clinical & social‑services data tracking, team‑based care implementation, and community‑clinical linkage development—and the vendor may be awarded for any combination of these strategies. The contract is a five‑year fixed‑price agreement (with two optional one‑year extensions) requiring a detailed line‑item budget, monthly usage reporting, and compliance with HIPAA, IRS 1075, and State data‑residency rules. • Deliver a statewide, data‑driven cardiovascular health learning collaborative and technical assistance to DPH. • Deploy EHR‑based SDOH screening tools, integrate with Epic and the Delaware Health Information Exchange, and coordinate referrals via UniteUs/FindHelp. • Provide staffing, training, and project‑management support to meet the $200‑$400 k annual budget ceiling and produce quarterly and monthly performance reports.
