Physical Therapy Services for Jefferson County Public Health Service

The County of Jefferson seeks one or more qualified providers to deliver Physical Therapy (PT) services on an as‑needed basis to patients of the Jefferson County Public Health Service. Services may be performed in the patient’s home or in an outpatient clinic under Medicare home‑care episodes. The contractor must respond within five days of a service request, supply qualified licensed PTs and Certified PT Assistants, comply with extensive New York State and federal health regulations, and submit monthly itemized invoices for payment within 30 days. The contract runs from award through December 31, 2028 with the possibility of two additional one‑year extensions, and requires substantial insurance coverage and record‑keeping. • Provide licensed PT and CPTA services both in‑home and clinic settings for Medicare‑eligible patients. • Meet a 5‑day response time, adhere to all NYS health statutes, and undergo OIG/Medicaid screening. • Submit monthly invoices; County pays within 30 days; no patient charges allowed. • Maintain $1 M‑$2 M commercial liability, $2 M professional liability, and other statutory insurance; retain records for 6 years.
