Health Promotion Advocates

The State of Delaware seeks a vendor to provide Health Promotion Advocates (HPAs) who will work face‑to‑face with Delaware residents to complete Screening for Life (SFL) and Health Care Connection (HCC) applications, promote enrollment in cancer‑screening programs (including WISEWOMAN), and perform related care‑coordination and reporting duties. The contract is a one‑year professional‑services award with the option to extend up to five additional one‑year periods, and the work is performed statewide from a Delaware‑based office or partner facility. • Provide on‑site HPAs to assist clients with application completion, data entry, and transportation tracking. • Submit 100 % fully completed applications, update state databases daily, and refer clients to patient navigators within 30 days of enrollment. • Deliver monthly usage reports and quarterly 2nd‑tier subcontractor diversity reports, complying with Section 508 and WCAG AA accessibility standards.
