Description

The State of Delaware’s Division of Public Health is seeking a single prime vendor to provide comprehensive patient‑navigation services for the state‑run Screening for Life (SFL) cancer‑screening program and the WISEWOMAN cardiovascular‑risk‑reduction program. Navigators will identify eligible clients, guide them through screening and diagnostic follow‑up, remove barriers to care, and ensure accurate data capture and reporting. The contract is a one‑year fixed‑price professional services agreement with the possibility of up to five one‑year extensions. • Core responsibilities: client identification, appointment scheduling, barrier assessment, pre‑authorizations, and coordination with Delaware Cancer Treatment Program and survivorship services. • Required staff: Licensed Practical Nurse, Registered Nurse, Community Health Worker, or licensed medical social worker; no subcontracting permitted. • Reporting duties: monthly navigation summary reports (due 15th of each month) and quarterly 2nd‑tier subcontractor reports (due 15th of the month after each quarter). • Compliance: HIPAA, Section 508, WCAG AA, IRS 1075 (if Federal Tax Information is involved), background checks, and data residency within the United States. • Financial terms: No dollar ceiling disclosed; vendors submit a detailed cost template; up to 25 % of funds may be held until satisfactory performance; prices must be fixed through October 15, 2026; indirect costs (F&A) allowable up to 15 %.

Budget estimate

$500,000 – $2,000,000

The solicitation does not disclose a budget ceiling; however, similar state‑wide patient‑navigation contracts typically range from $500,000 to $2 million for a one‑year period, depending on the number of clients served, staffing mix, and overhead. Assuming a moderate volume (≈2,000 screened clients) and an average cost of $250–$1,000 per client for navigation services, the contract could be valued in the low‑half‑million to low‑two‑million dollar range.
