The County of Santa Cruz is seeking a qualified contractor to provide comprehensive Hepatitis C Virus (HCV) linkage and navigation services for at‑risk populations during FY 2026‑2027. The contractor will screen individuals, coordinate confirmatory testing, facilitate medical appointments, support treatment initiation and adherence, distribute client incentives, collect outcome data, and collaborate with the County’s Public Health HCV program.
