Connecticut Ryan White Part B Core and Support Service

The Connecticut Department of Public Health (DPH) Ryan White Part B (RWPB) Program is seeking proposals from community-based organizations, HIV service organizations, Federally Qualified Health Centers, community health centers, local health departments, and other eligible healthcare providers. The selected providers will deliver eight core and seven support services within the nine Connecticut Council of Government (COG) Planning Regions. The services will be provided to support individuals living with HIV/AIDS. • The RWPB Program aims to provide comprehensive care and support services to individuals living with HIV/AIDS. • The program seeks to improve health outcomes and quality of life for individuals living with HIV/AIDS. • The selected providers will be responsible for delivering core and support services, including medical care, case management, and outreach services. • The services will be provided in the nine COG Planning Regions, which include Capitol, Greater Bridgeport, Lower CT River Valley, Naugatuck Valley, Northeastern, Northwest Hills, South Central, Southeastern, and Western. • The program is funded by the Ryan White Part B grant, and the selected providers will be required to comply with all applicable federal and state regulations.
