Food is Medicine Infrastructure Initiative

The State of Delaware’s Division of Public Health seeks a professional‑service vendor to design, implement, and sustain a “Food Is Medicine” (FIM) infrastructure in rural Kent and Sussex counties. The effort is divided into three components: (1) creation of payment‑infrastructure and research to enable CPT‑based billing and value‑based contracts; (2) deployment of technology solutions and staffing of community health workers and a registered dietitian to provide nutrition counseling and remote monitoring; and (3) training of culinary‑medicine teachers, piloting a six‑month intervention for 375 participants, and scaling best‑practice protocols. The contract is a fixed‑price, milestone‑based professional‑services agreement, with a one‑year initial term and up to four optional one‑year extensions. Required reporting includes monthly usage reports, quarterly subcontractor diversity reports, quarterly progress updates, and an annual impact report.
