The U.S. Army Corps of Engineers, Portland District, intends to solicit a firm-fixed-price service contract to procure a Fall Hazard Survey for Lookout Point Dam, Lowell, Oregon. The contractor will be required to perform a fall hazard survey, including a site visit, data analysis, and report preparation. The survey report shall include an executive summary, technical explanation, glossary, and regulatory requirements. The contractor shall also provide proposed solutions, including estimates for engineering, supply, and installation to remedy all hazards identified. The contract duration is expected to be 1 day, with scheduling to occur after contract award and within 60 days of award. • The contractor shall provide a fall hazardous survey for Lookout Point Dam, Powerhouse, and adjacent structure. • The contractor shall perform to the standards in the contract and comply with all local, state, and federal regulations during the services performed. • The contractor shall prepare a Job Safety Analysis, also known as Activity Hazard Analysis (AHA), in accordance with EM-385-1-1 and submit it as part of the Accident Prevention Plan for approval by the designated COR prior to initiation of the work tasks. • The contractor shall provide all necessary equipment, supplies, PPE, and materials to provide fall hazard survey site visit as well as the report that accompanies the survey. • A government representative shall accompany the contractor at all times.
