Speech Therapy Services for Jefferson County Public Health Service

The County of Jefferson is seeking a qualified provider to deliver comprehensive speech‑language‑swallowing (SLS) services to clients of its Public Health Service. The contractor must perform assessments, develop and update plans of care, provide direct therapy (in‑home or via telehealth), and maintain rigorous documentation and reporting in accordance with New York State and federal regulations. The contract is awarded on an hourly‑rate basis for a three‑year term with two optional one‑year extensions. • Conduct initial speech, voice, language, and swallowing assessments within 5 working days of referral. • Create a signed Plan of Care within 5 calendar days of the first visit and update it at least every 60 days or when clinical changes occur. • Deliver therapy services (home visits or two‑way audio‑video telecommunication) and provide client/family education, with progress notes submitted within 24 hours of service. • Submit twice‑monthly invoices detailing client name, activity time, hourly rate, and total charge; payments made within 30 days of receipt. • Meet licensing (NY State Dept. of Education), insurance, and OIG/Medicaid IG screening requirements, and undergo periodic supervision by the Public Health Service.
