Department of War Pharmacy Uniform Formulary Blanket Purchase Agreement / Uniform Formulary Additional Discount Program

The Department of War (DoW) under the Defense Health Agency is issuing an RFQ to obtain price quotations for newly‑approved pharmaceutical agents that will be placed on the Uniform Formulary (UF). Vendors must submit both Blanket Purchase Agreement (BPA) pricing (unit price per NDC, consistent across package sizes) and Additional Discount Program (ADP) pricing (non‑FAMP refund percentages and/or WAC‑based discounts). The solicitation supports two contract mechanisms – UF BPA for direct pricing and UF ADP for rebate/discount structures – and will be evaluated by the DoW Pharmacy & Therapeutics Committee at its November 4‑5, 2026 meeting. • Provide unit‑price quotes for each NDC listed in the Utilization Dataset; price must be ≤ current Federal Supply Schedule or “Big 4” prices. • Submit ADP calculations: non‑FAMP refund formula or WAC discount percentage, with required Quote Review Checklist and signed pages. • Pre‑quotation teleconference on 14 Oct 2026; all quotes due by 26 Oct 2026 12:00 PM CST. • Awards will be made under a Blanket Purchase Agreement with 180‑day quote validity and standard termination/price‑increase provisions.
