GROUP VISION INSURANCE RFP

Polk County is seeking a group vision insurance provider for its 1,600 employees. The selected firm will be responsible for claims administration, provider network, employer and employee services, reporting, and online enrollment capabilities. The proposal should include a brief history of the firm, service team description, experience in providing services to governmental entities, and a complete description of services to be provided. The evaluation criteria include administrative, financial, service, and technology aspects. • The firm must have at least 5 years of experience in providing services to similar sized public sector employers. • The firm must provide a fully-insured plan for vision insurance. • The firm must have a well-established provider network with competitive discounts. • The firm must provide enhanced customer service through communication plans and a user-friendly website.
