The State of Delaware Department of Health and Social Services, Division of Substance Abuse and Mental Health, is seeking proposals from qualified organizations to provide Community-Based Residential Alternative services in a Group Home setting to individuals within the state of Delaware enrolled in the Promoting Optimal Mental Health for Individuals through Supports and Empowerment (PROMISE) program. The goal is to further develop the community-based behavioral health service system to better serve individuals with severe and persistent mental illness and significant substance use disorders. Selected bidders will provide services in a home-like setting for 5 to 10 individuals requiring 24/7 care. Key aspects of the proposal include: * Providing Community-Based Residential Alternative services in a Group Home setting * Serving individuals with severe and persistent mental illness and significant substance use disorders * Offering person-centered, recovery-oriented services * Developing a written person-centered plan of care for each beneficiary * Ensuring services are individualized to meet each beneficiary's needs and goals * Providing strong links between systems to ensure a comprehensive and coordinated approach to services
