Caregiver Resource Centers

The State of Delaware is seeking a qualified vendor to establish and operate one or more Caregiver Resource Centers (CRC) that provide comprehensive information, counseling, training, referral, and support services to unpaid caregivers of older adults and persons with disabilities. The contract is a one‑year professional‑services award with up to four one‑year extensions, and the vendor must deliver both in‑person and virtual services, maintain a staffed center, manage case assessments, and meet extensive reporting and compliance requirements. • Operate a physical CRC with ≥ 20 hours/week of service, staffed by a part‑time coordinator and qualified support staff. • Deliver a suite of services including caregiver skills training, respite referrals, assistive‑technology assistance, counseling, support groups, and outreach/marketing activities. • Complete intake assessments (Form CF‑044), develop individualized service plans, submit monthly usage, financial, and diversity reports, and adhere to strict data‑residency, background‑check, insurance, and FOIA requirements.
