Advanced Planning for Individuals Receiving DDDS Services's HCBS

The State of Delaware’s Department of Health and Social Services seeks a vendor to provide advanced planning, legal‑decision‑making, and advocacy services for individuals receiving the Division of Developmental Disabilities Services (DDDS) Home‑ and Community‑Based Services (HCBS) waiver. The work focuses on creating supported decision‑making agreements, powers of attorney, and advance health directives, assisting with voluntary guardianship terminations, conducting outreach/education, and reporting satisfaction metrics linked to state performance indicators. The contract is for one year with the possibility of up to four one‑year extensions and short‑term month‑to‑month extensions. No subcontracting is permitted, and all work must be performed within Delaware. • Develop and deliver supported decision‑making agreements, POAs, and advance directives for up to 100 individuals per year. • Provide outreach, education, and bilingual (English/Spanish) materials on alternatives to guardianship. • Collect, analyze, and report service‑recipient satisfaction and program‑evaluation metrics, submitting monthly and quarterly reports. • Maintain required insurance, background‑check compliance, and data‑residency (U.S.) requirements.
