RFQ Speech Language Pathologist Services

The Tombstone Unified School District #1 is seeking a qualified Speech‑Language Pathologist (SLP) to deliver comprehensive therapeutic, assessment, and consultative services to K‑12 students across three district schools. The contractor must hold all Arizona licensure and certification requirements, maintain fingerprint clearance, and be capable of billing Medicaid school‑based claims. Pricing must be itemized, including transportation costs between sites. • Provide individual and group speech‑language therapy, screenings, and ongoing documentation via e‑IEP Pro. • Conduct assessments, develop IEP goals, and participate in annual IEP meetings and multidisciplinary team consultations. • Offer training and support to teachers, families, and community partners, and coordinate with any district‑hired SLPA staff. • Submit detailed hourly/daily rates, quantity estimates, taxes, travel fees, and any discounts in the proposal.
