Rural Community Health Hubs

The State of Delaware’s Division of Public Health is seeking a qualified vendor to design, implement, and manage a coordinated network of Rural Community Health Hubs that includes both mobile health units and fixed‑site health centers serving underserved rural areas of Kent and Sussex Counties. The effort requires procurement, staffing, technology integration, community partnership development, data collection, quality improvement, and a sustainability plan to ensure long‑term operation. • Design and operate mobile health vans equipped for primary, preventive, behavioral‑health, specialty, and telemedicine services. • Establish or expand brick‑and‑mortar health hubs with multidisciplinary teams, EMR/DHIN integration, and community outreach calendars. • Provide comprehensive reporting, closed‑loop referral tracking, and performance metrics aligned with state health‑equity goals. • Deliver a detailed work plan, budget, and sustainability strategy that leverages diversified revenue streams and partnership models.
