The State of North Dakota’s Department of Health and Human Services is seeking information from vendors about comprehensive, provider‑facing solutions that enable population‑health management, care coordination, and seamless interoperability across disparate electronic medical record (EMR) systems. The RFI aims to gather market data on technology platforms, analytics, patient‑engagement tools, and optional advisory or implementation services that could support rural and tribal providers, enhance real‑time clinical interventions, and improve performance under value‑based contracts. Respondents are asked to describe capabilities, delivery models, integration approaches, and indicative pricing, but no contract award or budget has been defined at this stage.
• Core capabilities sought include data exchange & normalization, analytics/APCD reporting, closed‑loop referral and care‑coordination workflows, and provider‑facing patient‑engagement/telehealth tools.
• The State expects solutions that can integrate with existing statewide infrastructure (HIE, APCD, PDMP) using standards such as HL7 v2, FHIR, C‑CDA, X12, Direct, APIs, etc., and that support person‑level identity resolution and consent management.
• Delivery models may be single‑platform, best‑of‑breed, prime‑plus‑subcontractor, or consortium; advisory, implementation, and program‑management services are also welcome.
