Physical Therapist RFP March 2026

The Nyack Union Free School District is seeking a qualified provider to deliver comprehensive physical therapy (and occupational therapy) services to students across six school sites for the 2026‑2027 academic year. The contractor must staff licensed New York State physical therapists, manage all documentation and Medicaid reporting, supply necessary therapy materials, and handle travel between schools. The contract is a one‑year fixed‑price agreement with potential annual extensions tied to CPI adjustments. • Provide individual and small‑group PT/OT sessions (max 5 students per group) per student IEPs. • Complete screenings, evaluations, progress reports, and mandatory school‑team participation. • Maintain full compliance with licensing, background checks, certifications, insurance, and COVID‑19 protocols. • Submit hourly‑rate pricing (Exhibit A‑7) and a full suite of required certifications, affidavits, and insurance certificates.
