Palm Beach County’s Community Services Department seeks an existing, commercially‑available SaaS platform to digitize client engagement and care coordination for residents with behavioral health, substance‑use, and other chronic conditions. The contractor will configure, customize, integrate, implement, train, and provide ongoing support for a county‑wide solution that improves medication adherence, appointment attendance, and reduces emergency‑room utilization. The contract runs 19 months beginning Dec 1 2026 with two optional 12‑month renewals. Key milestones include a question‑submission deadline (Oct 29 2026), proposal deadline (Nov 12 2026), selection committee meeting (Nov 24 2026), award notice (Jan 12 2027), and contract start (Jan 12 2027).
