Description
The Navajo Nation’s Special Diabetes Program seeks qualified vendors to supply, deliver, and install a full suite of fitness equipment at the Kayenta Wellness Center in Kayenta, AZ. The scope includes cardio machines, strength‑training equipment, free‑weights, accessories, and a complete on‑site installation with supervision and warranty certification. Vendors must provide detailed cost breakdowns, equipment layout plans, and required certifications (debate, non‑collusion, subcontractor responsibility). The contract will be awarded to the best overall proposal based on capability, approach, and price.
• Supply and installation of cardio and strength‑training equipment (treadmills, bikes, ellipticals, weight machines, free‑weights, etc.)
• Delivery of accessories and ancillary items (balance pads, yoga blocks, TRX gear, etc.)
• On‑site supervision of installation, start‑of‑warranty documentation, and training as required
• Required documentation includes equipment list, floor plan, IRS Form W‑9, and Navajo Nation certification forms.
