Collaborative Facilitation

The State of Wyoming is seeking a contractor to provide collaborative facilitation services for county collaboratives developed for Plans of Safe Care. The contractor will be responsible for facilitating monthly collaborative meetings, monitoring the process, and disseminating best practices. The contractor will work with the Wyoming Department of Health and the Wyoming Department of Family Services to support the development of collaboratives across the state. The contract will be for a period of up to four years, with the option to extend for an additional two years. • The contractor will be required to have personnel who can manage and facilitate collaboratives within a region. • The contractor will be required to provide a project manager to represent and oversee the project. • The contractor will be required to submit a work plan that meets the needs of the RFP and indicates a thorough understanding of the scope of work. • The contractor will be required to provide monthly management and budget reporting, as well as customized reports of collaborative data as requested. • The contractor will be required to enter into a Business Associate Agreement (BAA) with the Wyoming Department of Health to ensure the confidentiality of protected health information.
