DSS Rural Health Transformation Project Management

The State of South Dakota is seeking a qualified vendor to provide integrated program‑management services for its Rural Health Transformation initiative, covering three grant‑funded components: Medicaid Primary Accountable Care Transformation (PACT), Medicaid Rural Health Access & Quality Grants, and Behavioral Health. The contractor will act as staff augmentation under State direction, handling coordination, reporting, risk management, and grant‑administration support for a two‑year base period with up to four one‑year extensions. The effort is expected to be 40‑80 hours per week (approximately 1 FTE) and must adhere to extensive bi‑weekly, monthly, and quarterly reporting requirements, including CMS reporting. • Provide overall project‑management, timeline, and risk coordination across the three initiatives. • Deliver bi‑weekly internal updates, monthly work‑order status reports, and quarterly/annual CMS reports. • Support grant‑administration functions for the Rural Health Access & Quality Grants (application tracking, award monitoring, technical assistance). • Assist with PACT model design coordination, stakeholder engagement, and data preparation for reporting. • Minimal travel (up to four on‑site meetings per year); all travel costs included in hourly rates.
