Description
The Department of Veterans Affairs is seeking a qualified contractor to replace the flooring in Operating Room #5 at the Des Moines VA Medical Center. The work includes demolition of existing floor and sub‑floor, asbestos survey and abatement, removal of debris, and installation of new sheet‑vinyl resilient flooring with integral cove base to match existing floor elevation. The contractor must adhere to VA safety, infection‑control, and security requirements, provide all necessary permits, maintain negative‑pressure containment for asbestos work, and deliver a one‑year warranty.
• Demolition to deck, asbestos testing/removal, and proper disposal of hazardous waste.
• Supply and install VA‑specified sheet‑vinyl resilient flooring (PG‑09‑65‑16) with required underlayment, primers, and adhesives.
• Provide complete safety documentation (APP, AHAs, SDS, TB screening), secure VA PIV cards for all personnel, and follow strict work‑hour windows (evenings/weekends).
