Employee Ancillary Benefits - Dental

This contract establishes a three‑year, self‑insured dental benefits program for all eligible Sedgwick County employees and their dependents. Delta Dental of Kansas, Inc. will administer the plan, maintain the existing network design, process electronic claims, provide member portals, and handle all enrollment and reporting functions. The County will fund the plan at a guaranteed rate of $3.35 per employee per month (PEPM) with a 90 % County / 10 % employee cost split, and the agreement includes two one‑year renewal options. • Scope: full administration of a dental benefit plan (claims processing, enrollment, network management, reporting). • Financial terms: $3.35 PEPM rate guaranteed for three years; County covers 90 % of total cost. • Duration & renewal: 1 Jan 2022 – 31 Dec 2024 with two optional one‑year extensions.
