PERSON-CENTERED TRAINING AND ADVANCE DIRECTIVES

The State of Wyoming, Department of Health, Behavioral Health Division is seeking a vendor to provide person-centered training and advance directives. The selected vendor will provide in-person training at the Wyoming Life Resource Center and the Wyoming State Hospital, as well as collaborate with facility staff to update policies and forms. The training should focus on person-centered care, effective person-centered planning, strategies for person-centered goal development, and methods for assessing goal achievements. The vendor should also provide a recorded presentation of the training for future use. • The vendor should have experience in providing person-centered training and advance directives. • The vendor should be able to provide a comprehensive training program that meets the needs of the Wyoming Life Resource Center and the Wyoming State Hospital. • The vendor should be able to collaborate with facility staff to update policies and forms. • The vendor should be able to provide a recorded presentation of the training for future use. • The vendor should be able to provide a project manager to oversee the project. • The vendor should be able to provide a work plan that meets the needs of the RFP and indicates a thorough understanding of the scope of work.
