Mohave Valley Elementary School District #16 is seeking a qualified, licensed Physical Therapist (or agency) to deliver school‑based physical therapy services to approximately ten students identified in Individualized Education Programs. The contractor will provide screening, direct therapy (in‑person and/or tele‑therapy), evaluations, IEP meeting participation, goal development, data entry, and monthly billing. All equipment, supplies, and necessary licensure documentation must be supplied by the contractor, and services are billed on a fee‑for‑service, hourly‑rate basis. The contract is initially for one year with the possibility of up to four additional one‑year extensions. • Provide 30 minutes/week of therapy per student (≈5 hrs/week total) plus screenings, evaluations, and IEP support. • Supply all therapy equipment, testing materials, and (if needed) hardware for tele‑therapy; no district‑provided equipment. • Submit monthly invoices and student‑contact forms; billing is strictly to the district (no Medicaid/insurance). • Must meet extensive compliance requirements (Arizona licensure, insurance, fingerprint clearance, EDGAR, forced‑labor certification, etc.).
