Post Release Case Management

The State of Delaware Department of Correction is seeking professional services to provide post-release case management statewide to individuals leaving custody from Level V and IV institutions. The selected provider will enter into a contract with the DDOC to provide post-release case management services, which will include brief assessments of reentry needs, establishing reentry goals, and measuring reentry progress over time. The provider will work with each client to create a care plan that addresses the client's immediate post-release needs and long-term transitional period in the community. The contract will be for a period of three years with three optional extensions for a period of one year each. Key aspects of the job include: • Providing post-release case management services to individuals with high needs and high risk upon release from prison • Collaborating with various stakeholders, including the Bureau of Substance Abuse, Mental Health, and the Delaware Treatment Access Center • Maintaining a caseload of 30 clients and providing regular progress updates • Utilizing evidence-informed case management models and incorporating motivational interviewing techniques and cognitive-behavioral interventions • Ensuring adequate length of case management support and increasing motivation to engage in prosocial activities
