Family Wrap Hawaii (FWH) Services

The Department of Human Services is seeking proposals for Family Wrap Hawaii Services to support families with complex needs. The services include facilitation and plan development, community navigation, peer parent supports, peer youth supports, and flexible funds. The contract is expected to begin on December 28, 2025, and the proposal submission deadline is December 9, 2025. • The proposal should include a complete budget and a narrative addressing the requirements specified in the RFP. • The applicant should have experience in providing similar services and demonstrate the ability to meet the requirements. • The evaluation process will include a review of the proposal's content, experience, capabilities, and financial information. • The contract will be awarded based on the most advantageous proposal to the State. • The provider will be required to participate in the State's future development of a service delivery plan. • The provider will be required to submit quarterly activity reports and expenditure reports. • The provider will be required to maintain accurate documentation of service units delivered to clients and contract expenditures.
