Contracted Case Management

Dakota County’s Community Services Division is seeking qualified community agencies to provide comprehensive case‑management services for individuals with disabilities and seniors across multiple waiver and non‑waiver programs (CADI, CAC, BI, DD, EW, AC, Rule 185, VA/DD). The contractor will be responsible for client outreach, service‑plan development, coordination, documentation in the county’s SSIS system, and compliance with HIPAA, MGDPA, and WCAG 2.1 AA accessibility standards.
