Mohave Valley Elementary School District #16 is seeking a licensed Arizona physical therapist (or agency) to provide comprehensive therapy services for its student population during the 2025‑2026 school year. The contractor will deliver in‑person and tele‑therapy, conduct screenings and evaluations, attend IEP meetings, produce required documentation, and handle travel between the district’s three schools. All services must be covered by a single flat‑rate proposal; no hourly pricing is allowed.
