Project Management Support for Chronic Disease Management Initiative

The South Dakota Department of Health is seeking a vendor to provide full‑service project‑management support for its Chronic Disease Management Initiative under the Rural Health Transformation program. The contractor will act as the administrative hub for two upcoming grant streams, handling contract development, liaison duties, fiscal oversight, data collection, reporting, and evaluation support for funded health entities and schools.
