Mobile Integrated Healthcare Implementation Services

The Vermont Department of Health is seeking Letters of Interest from qualified Emergency Medical Services (EMS) agencies and hospital-based EMS programs to implement Mobile Integrated Healthcare (MIH) services. The goal of the program is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce, and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. The selected MIH sites will be geographically distributed to support access to services across rural Vermont and shall provide, at a minimum, one of the three core MIH services related to Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Joint Replacement Care. Key aspects of the program include: • Implementing MIH services at five implementation sites • Providing core MIH services, such as in-home clinical assessment and treatment, medication reconciliation, and disease-specific education • Incorporating telehealth or virtual consultation components, if desired • Ensuring compliance with applicable State and Federal laws, including HIPAA • Maintaining a structured Quality Assurance process • Providing a high-level budget and estimated number of unique people served The selected applicants will be expected to sign a grant agreement with the State, including standard grant agreement and Attachment C.
