The Department of Veterans Affairs is seeking a contractor to provide temporary housing and supportive services for homeless veterans in the Lexington, Kentucky area. The contractor will be responsible for providing a safe and supportive environment, including room and board, case management, and assistance with daily living activities. The contractor will also be expected to develop and implement an Individual Service Plan (ISP) for each veteran, which will include goals and objectives for permanent housing and other supportive services. The contract will be for a base year with four option years. * The contractor will provide temporary housing for up to 20 veterans, with a minimum of 12 beds available at all times. * The contractor will provide case management services, including development of an ISP for each veteran. * The contractor will provide assistance with daily living activities, including food, laundry, and transportation. * The contractor will provide a supportive and therapeutic environment, including group activities and individual instruction. * The contractor will work with VA staff to ensure that veterans receive necessary medical and mental health services.
