Addendum Three: RFQQ SQM Questionnaire

The City of Detroit Demolition Department is seeking proposals from pre-qualified vendors for the abatement and emergency alteration/demolition of residential/commercial structure(s). The successful vendor must complete all services sought through this RFP in accordance with the associated Scope of Services. The City will evaluate proposals based on experience, capacity, project plan, project schedule, and cost proposal. • The RFP includes a preliminary schedule based on the expected date of the Notice to Proceed. • The City reserves the right to amend, change, or modify the schedule. • The successful vendor must secure and submit both a payment and performance bond in the amount of twenty-five percent (25%) of the total contract value. • The successful vendor must submit certificates of insurance that include endorsements naming the City of Detroit and the Detroit Land Bank Authority as additional insureds. • The City will apply equalization credits as applicable under City ordinance prior to tabulating cost proposals.
