Surveyor Services

The Wisconsin Department of Health Services, Division of Quality Services is issuing an RFP for Surveyor Services to investigate complaints and conduct recertifications at Long‑Term Care (LTC) and Non‑Long‑Term Care (NLTC) health facilities statewide. The solicitation is split into two lots—Lot 1 for LTC facilities and Lot 2 for NLTC facilities—and vendors may bid on either or both. Up to three contracts may be awarded per lot. Proposals must clearly identify the lot being targeted. • Scope: Initiate investigations of complaints, perform on‑site surveys, and complete recertification documentation for LTC (including ICF/IID) and NLTC (including hospitals and ESRD) facilities. • Delivery: Statewide coverage across Wisconsin; multiple surveys per contract period. • Submission: Proposals due after the available date of 10/29/2026; questions must be submitted by 12/09/2026. • Evaluation: Results will be used to award up to three contracts per lot. • Contact: Wendy Reince, Procurement, DHS (email and phone provided).
